Sport Compilation Attestation/Check List Requirements

Important Info for Programs/Associations & Facilities

Attached you will find the state required checklists and attestation forms that you and/or your facilities are required to complete. While we are still expecting additional guidance and clarification from the state pertaining to a number of areas, these checklists should be considered as accurate and complete. 

Please share this information with your coaches and managers, as it is likely that they will be serving as the “Activity Organizer” at times on behalf of the program. Programs may want to consider assigning parents the task of “Activity Organizer” on a rotating basis to ensure your program remains compliant with the guidance. We strongly encourage communication between the program and facility regarding responsibilities. 

The forms should be filled out as follows: 

Sport Compliance Attestation: Program and Facility 

Activity Organizer Checklist: Whoever is purchasing/using the ice 

Facility Operator: Rink Management 

It is strongly recommended that any and all Massachusetts Hockey programs complete the Sport Compliance Attestation and the Activity Organizer Checklist and post them on their website so that they are easily accessible should they be requested by any local Board of Health. Thank you all for your cooperation and commitment to keeping the rinks a safe place for our players and families to enjoy youth hockey!
